New Patient Registration Questionnaire
Patient Questionnaire (English)
Please complete the patient questionnaire and bring it to the surgery along with your registration form. If possible please bring your NHS medical card. If you do not have a medical card, you will be asked to complete a registration form for each person registering with the Practice.
Proof of address will be required, so please ensure that you bring along a copy of a recent household bill confirming your name and full postal address.
For additional information we have a medical registration form as means to gather immediate medical information which will assist us until we receive your medical records from your previous practice. (Insert medical registration form here)
As a new patient you will be invited to attend for a 'new patient health check' with our practice nurse or healthcare assistant.
The practice serves the area of Gorseinon and its immediate boundaries. Due to registration demands, we only accept patients who live within our practice area. If you move out of the area you will be required to register with a new practice near to your new home address.
