Access to Patient Information

The Individual Health Record (IHR) has been introduced in Wales. This means some of your information will be shared with doctors/nurses working for the Swansea Out of Hours Service (SOOH) who are responsible for your care outside surgery hours.

The IHR holds limited patient details and the SOOH doctor/nurse must obtain patient consent before looking at any patient information. Please be assured all professional staff must adhere to the Data Protection Act at all times.
